Gujarat · outpatient and inpatient care

From the first visit to the next clinician, one case travels with you.

Swasthora helps patients in Gujarat find a suitable first doctor, then carry a shared history through referrals, labs, and imaging — including people who start offline.

How it works

Phase 1 is a traditional web app: pages, forms, and calendars. You describe symptoms, choose a doctor, and keep every handoff on one case.

  1. Step 1

    Describe what you feel

Start with structured symptoms: how long they have lasted, how severe they are, and which part of the body is involved.

  2. Step 2

    Find a nearby first doctor

See clinics and doctors in Gujarat matched by specialty, OPD or IPD, open slots, and distance. Hours and location are shown before you book.

  3. Step 3

    Open one shared case

Booking creates a case. The first doctor on that case documents the visit, then refers you onward when the next clinician should take over.

  4. Step 4

    Keep the chain going

You book or reschedule the next appointment, or a clinic books for you. Offline doctors can join the same case when you share the case link.

Who it is for

Patients

One account across clinics. You can see every section of your case, follow referrals, and close a case when that episode of care is finished.

Doctors

Document your own section in free text or PDF, including diagnosis, treatment, and IPD admission details. Refer to the next doctor or to clinical staff.

Clinical staff

Receive investigation referrals, see the tests that were requested, and upload reports with notes. Each person edits only their own section.

Clinics and hospitals

Standalone clinics and hospital clinics share the same directory. Admins approve who may appear for public booking. They see schedules, not clinical notes.

Walk-ins and offline doctors stay on the same case.

A clinic QR lets a walk-in patient create an account at the desk. A case-report link lets an offline doctor or clinical staff member join, write their section, and upload a report. The patient can revoke that link.

  • Appointments live on the doctor’s calendar, up to six months ahead. Patients can reschedule. Closing a case cancels future visits on it.
  • Notices for new appointments, referrals, uploads, and closures stay inside the app. Case history is limited to the people assigned to that case.
  • Access is protected in transit and at rest, with row-level rules so a clinician sees a case only when they are on it.
